The cheapest route is coverage, near $25 a month. Without it, the approved oral Wegovy tablet at $149 is now cheaper than most compounded injectable programmes once membership is added — a reversal from two years ago that most comparison pages have not registered.
Ranked on twelve months, not the first month
| Route | Monthly | Twelve months | FDA-approved? |
|---|---|---|---|
| Commercial coverage plus savings card | $25 | $300 | Yes |
| Medicare GLP-1 Bridge, if eligible | $50 | $600 | Yes |
| Oral Wegovy tablet, self-pay | $149 | $1,788 | Yes |
| Compounded, flat plus membership | $178 | $2,136 | No |
| Compounded, dose-tiered entry rate | $99–$299 | $1,200–$3,600 | No |
| Brand Wegovy, NovoCare direct | $349 | $4,188 | Yes |
| Brand Wegovy via telehealth | $298–$348 | $3,576–$4,176 | Yes |
The reversal nobody has priced in
For two years the argument for compounded semaglutide was simple: the approved drug cost over a thousand dollars a month and the compounded version cost two hundred. That gap has closed from both ends.
Brand Wegovy fell to $349 through NovoCare. The approved oral tablet arrived at $149. Meanwhile compounded programmes commonly land at $178 to $199 all-in once a membership is added, and several are dose-tiered so the entry rate is not the maintenance rate.
An approved product with its own trial programme is now the cheaper option for many buyers. That is the single most useful fact on this page and it appears on almost no comparison site, because most were written when the opposite was true and have been updated only on price, not on conclusion.
Where the low compounded figures come from
Figures in the $99 to $149 range are real but usually conditional. In our experience checking them, they are one of four things:
- An annual prepayment. One provider's $99 is billed yearly at $1,188 up front.
- Medication only. A $99 rate with a mandatory $79 membership is $178.
- A starting dose. Dose-tiered programmes quote the lowest step.
- A first-month promotion. One provider's page shows $99 first month against $280 ongoing.
We withdrew eleven such figures in July after finding they did not survive checking. Two later came back when we found them on the providers' own pages; the rest did not. The record is in our corrections log.
The salt-form question, unique to compounded semaglutide
Semaglutide sodium and semaglutide acetate are not the same active ingredient as the semaglutide base in the approved products, and the FDA has said they are not appropriate for compounding. Ask for the exact salt form and concentration in writing before you buy.
What the cheapest route costs you in evidence
Every efficacy figure in this field — STEP, SELECT, SURMOUNT — comes from a trial of an FDA-approved product. None was collected on a compounded preparation, a microdose or an orally disintegrating tablet.
That does not make compounded preparations ineffective. It means the evidence you are being shown was gathered on something else, and the strength of that evidence does not travel with the molecule across a change of formulation, dose or route.
At $149 for an approved pill against $178 for a compounded injection, the trade is no longer cost against evidence. It is a preference about route.

|---|---|---|
| Starting dosage | 2.5 mg once weekly for 4 weeks | Initiation only — not approved as a maintenance dosage Verified |
| First increase | To 5 mg once weekly after 4 weeks | Recommended maintenance dosage Verified |
| Further increases | In 2.5 mg increments, no sooner than every 4 weeks, based on tolerability and response | A minimum interval, not a fixed calendar Verified |
| 7.5 mg and 12.5 mg | Available strengths used during titration | Titration steps, not recommended maintenance dosages Verified |
| 10 mg | Once weekly | Recommended maintenance dosage Verified |

| Above 15 mg | No approved dosage exists | Verified Verified |
| Program type | What it covers | Comparable with |
|---|---|---|
| Starter program | Introductory period, often lower doses | Other starter programs only |
| Ongoing program | Standard continuing supply | Other ongoing programs only |
| Maintenance program | Post-titration supply, often a fixed dose | Other maintenance programs only |

| Month-to-month | Cancellable each cycle | Other month-to-month plans only |
| Microdose program | Sub-therapeutic dosing outside trial evidence | Other microdose programs only |
Questions readers actually ask
What is the cheapest semaglutide?
Coverage, at roughly $25 a month. Without it, the approved oral Wegovy tablet at $149 is now cheaper than most compounded injectable programmes once membership is added.
Is compounded semaglutide still cheaper than brand?
Often not. Brand Wegovy fell to $349 through NovoCare and the approved oral tablet is $149, while compounded commonly lands at $178 to $199 all-in.
Why do some sites show $99 semaglutide?
Usually one of four things: an annual prepayment, a medication-only price with a separate membership, a starting dose on a tiered programme, or a first-month promotion.
Does trial evidence apply to compounded semaglutide?
No. STEP, SELECT and SURMOUNT all studied FDA-approved products. No trial has tested a compounded preparation, a microdose or an orally disintegrating tablet.

Capture standing: 35 of 86 price records in this dataset were read at the source by us: 24 from NexLife and 6 from Lilly's own pages for brand Zepbound. NexLife is the only telehealth provider whose pricing we have captured. Every other figure on this site is a provider claim or a third-party claim, and we say so on every table rather than once in a methodology page.
