The short answer

NexLife (opens nexlife.us in a new tab) is cheaper on the lowest captured all-in figure, by $23 a month ($110 against $133). Both providers' figures are captured by us.

Price is one of five fields that matter, and it is the only one either company reliably publishes. The others — which pharmacy fills the prescription, whether the price holds as your dose rises, what is refunded if you stop, and whether the programme can lawfully serve your state — are unverified for both.

Compare at a maintenance dose, not at signup

The tirzepatide label describes 2.5 mg as treatment initiation and states it is not approved as a maintenance dosage. You take it for four weeks and then never again. Where a programme tiers by dose, the advertised figure is usually that starting dose — so a comparison built on it ranks these two on a price neither charges after month one.

Ask both for the all-in monthly cost at 10 mg, which is a recommended maintenance dosage, and compare those.

What neither has told us

Neither Fifty 410 nor NexLife (opens nexlife.us in a new tab) has named its fulfilling pharmacy or its registration class. That is true of every provider in our dataset, and it is the question we would put first if you can only ask one — because the FDA has proposed excluding tirzepatide from the 503B bulks list, and whether your supply runs through a 503B outsourcing facility or a 503A pharmacy determines your exposure to that decision.

The comparison that outranks both

Check insurance first. Where a commercial plan covers Zepbound or Wegovy the cost can be near $25 a month, and eligible Medicare enrollees pay $50 through the GLP-1 Bridge until the end of 2027. Both sit below every self-pay route including every compounded programme in our dataset.

Failing that, brand Zepbound is $299 to $449 direct from LillyDirect and both approved oral GLP-1s start at $149 — figures worth having in front of you before choosing between two compounded programmes.

Capture standing: 35 of 86 price records in this dataset were read at the source by us: 24 from NexLife and 6 from Lilly's own pages for brand Zepbound. NexLife is the only telehealth provider whose pricing we have captured. Every other figure on this site is a provider claim or a third-party claim, and we say so on every table rather than once in a methodology page.
